Policies and Procedures for Submitting Request

Submit all donation requests online; written, faxed, and/or emailed requests are not accepted.
Complete the form in its entirety for consideration; DDM discards incomplete forms.
Organizations may receive a maximum of one donation per calendar year.
For consideration, DDM must receive completed forms at least four weeks prior to the organization’s event.
Non-profit organizations must have 501(c)(3) tax exempt status.
The organization’s Tax Exempt ID number is required. If the request is from a business sponsoring a charitable event, a member of the 501(c)(3) charity should complete the form using their number.
Completion of the form does not guarantee a donation is forthcoming; organizations should plan accordingly.
